Abstract
The published version had copy editing errors to several affiliations with one affiliation for David Hamer completely left off and one affiliation for Tom Solomon duplicated and attributed to Igor Koralnik. As a result Drs. Hamer, Koralnik and Solomon had problems with their affiliation. Additionally, the affiliation for Drs. Villanueva-Colina, Tovar and Davila Luna were supposed to be the same as Dr. Garcia at the School of Public Health at Universidad Peruana Cayetano Heredia. Lastly, requested queries in regards to tables 1 and 2 which were uploaded along with the proofs were not applied. Below are the correct affiliations. Hanalise V.
